This form when submitted with required information and proof of purchase entitles the purchaser to a $50 rebate, as follows:

1. This rebate offer only applies to new purchases of a CinemaView Duo dual display extender beginning from August 9, 2010. Duo units purchased prior to August 9, 2010 are not eligible for this rebate program.

2. The rebate form is available as a PDF download here. That form must be downloaded, printed, and fully executed.

3. The rebate form must be mailed to CinemaView and must include a copy of the store receipt or shopping cart receipt showing the purchase of a CV Duo on or after August 9, 2010. Copies of the store receipts or shopping cart receipts showing the purchase of two CV24 LCD displays must also be included. The two CV24 displays may have been purchased on different dates prior to or on the date of the CV Duo purchase.

4. The completed rebate form and the three receipt copies must be mailed to us in an envelope marked "Duo Rebate" to this address:

5. Upon receiving your mailed rebate form and receipts we will issue a rebate check for $50 and send it to you by first class mail. Please allow 2 to 4 weeks for processing. Limited to one rebate per household.
